The forms available on these pages are either in Adobe pdf format or are Microsoft Word forms or both. PDF files require Adobe Acrobat Reader to access.
Adobe pdf FilesWhen you follow a link to a specific form stored in pdf form, your computer will launch Acrobat Reader and open the selected form. You may then save the form to your own computer, print it, and then complete the form. If you do not have Acrobat Reader, follow this link to Adobe's download site then follow the instructions there for downloading and installing Acrobat to your computer. Files in this format must be completed by hand.
Microsoft Word Form FilesThese files have been converted to Word Forms. Files in this format may be completed by hand or onscreen, it is your choice!
When you click on a link to a specific file stored as a Word Form, what happens will depend upon whether you are using Internet Explorer Windows or Internet Explorer Macintosh:
Windows
Click on the link
The file will open in a window within Internet Explorer.
You can now complete the form but it may be easier if you
Right-Mouse click on the link
choose Save Target As from the pop-up contextual window
save the file to your computer
open the file in Word and complete it
Macintosh
Click on the link
The file will be saved to your computer
open the file in Word and complete it
In either case, the blanks in the form are defined as fields in Word and appear shaded. Thus, you may complete the form onscreen from within Word (press TAB to move from one field to the next), save the completed form to your own computer (use a different filename), and then submit the form as an email attachment to the appropriate office.
Microsoft Excel Template Files
These files have been converted to Excel files and saved as an Excel template. Files in this format may be completed by hand or onscreen, it is your choice! Formulas and functions have been inserted as needed so that calculations are automatic. In either case, you should save the expense template to a location on your hard drive and then SAVE AS and give it another name. By doing this, you will now have you own copy of the form on your computer and another working copy to complete for the expense reimbursement.
